Please be sure to follow the directions included on each form to assure that all required information and signatures are filled out before the form is returned to the Registrar's Office on the first floor of the Administration Building.

Sequencing guides and program checksheets check sheets detail all required courses for each specific major, including major, concentration and general education requirements. These guides or checksheets check sheets are available for all traditional, undergraduate students who were accepted to Springfield College into a matriculated program beginning with the 2002-2003 academic year. -
Registration Planning Form
Registration Planning forms are available to currently registered students via PrideNet. Accessing the form through PrideNet allows for the inclusion of personalized data. You may also print a generic copy of the form from this link as needed. Spring 2011 Technology Request Form
In order to better support your classroom technology needs, we ask for your input as we begin the process of assigning classrooms. Please use the form to help identify the type of equipment you need for each of your courses. Your request(s) can be more readily honored if submitted to the Registrar's Office no later than Nov. 12, 2010. Official Transcript Request Form
The Registrar's Office cannot accept faxed requests for transcripts nor can transcripts be transmitted to a destination by fax. For more information on transcripts, call the Office of the Registrar at (413) 748-3393. A $5.00 fee for each transcript copy is required. Payment in the form of a personal check or money order made payable to Springfield College should accompany the request.
Unofficial Transcript Request
This form is used to request an unofficial transcript, which includes the complete academic record of the student’s undergraduate and/or graduate coursework. This transcript will be stamped “UNOFFICIAL”, and “ISSUED TO
STUDENT”, and will not bear the signature or name of the Registrar, and will not bear the college seal. An unofficial transcript is issued to the student for their personal and/or academic use.

Change of Address
Note that current students may also change their own address information via the Campus Web under My Profile tab in the Personal Information section.

Degree Application
This form must be completed by all students in order to be eligible to graduate. Applications are due into the Registrar's Office by February 8th for students graduating in May or August, and by October 12th for students anticipating graduating in December.

Nonmatriculated Graduate Data Form (.pdf)
This form must be completed prior to meeting with the Assistant Vice President of Academic Affairs. Be sure to fill out the form completely and provide all notated information and supporting documents. The student should also print out and complete an Add Drop form, listing tentative courses, to be approved by the Assistant Vice President.

FERPA Release Form.pdf
Springfield College will maintain student confidentiality rights and protect access to information as provided by the Family Educational Rights and Privacy Act (FERPA). Except as provided by law, information from a student’s records will not be released without the prior written consent of the student. This legislation also provides numerous exceptions whereby the College may release information without prior notice and/or consent of the student. Please refer to the College Catalog for more information.

Change of Major (Undergraduate)
This form is to be used for any changes to an undergraduate student’s major, minor, concentration and / or advisor. It must also be used if a student is dropping a 2nd major, any minor or any concentration. Additional information is noted on the form.

Change of Major or Program (Graduate)
This form is to be used for any changes to a graduate student’s major, program of study and / or advisor. Additional information is noted on the form.

Course Substitution or Waiver Request
Students should use this form to request a substitution or waiver of any major, minor, General Education, or program requirement or selective.

Graduate Student Petition for extension of candidacy
A time limit of five years from the initial registration within the Masters Degree or Certificate of Advanced Graduate Study program is allowed for the completion of all requirements for the degree. If a student has not completed his/her program within the five year period, their academic record will be closed out and it will be necessary to re-apply to the program. In special circumstances, a student may petition to extend this period of candidacy.

Waiver of Residency Request-Traditional Students
Fulfillment of the residency requirement calls for the completion of both a minimum of 60 career semester hours at Springfield College and 15 of the final 30 semester hours required for graduation at Springfield College.
Add/Drop Form
This form must be used to add a course within the designated add period. During the first five days of each semester, students may request additions to their schedule of courses. Students must complete an Add/Drop Form to complete schedule additions. All Add/Drop Forms must be signed by the student’s advisor before they are brought to the registrar for processing. After the five-day add period, students may not make additions to their academic schedules. During the first ten days of the semester, students may drop a course they have registered for. An approved Add/Drop Form must be signed by the student’s advisor. Courses dropped during the first ten days of the semester (excluding summer) will not be reflected on the student's academic record. Students are urged to refer to the Academic Schedule each semester for more specific information.

Course Withdrawal Form
Students, with the permission of their advisors and course instructors, may withdraw from a course at any time up to the point at which eighty percent of the course is completed. Courses dropped after the first ten days of the semester (excluding summer) will be reflected on the student's academic record with a grade of 'W'. The end of the twelfth week is the last day to withdraw from a course that meets the entire semester. Students are urged to refer to the Academic Schedule each semester for more specific information. -
Course by Arrangement
A Course by Arrangement (CBYA) is requested by a student when a course in the Springfield College catalog is not offered during the current semester or session. The CBYA is subject to approval by the appropriate faculty member, department Chairperson, the student’s advisor and the Dean of the School. -
Guided Individual Study/Research
A Guided Individual Study/Research course is requested by students who wish to further develop their personal and professional needs by conducting research while working independently under a faculty supervisor. These courses include RSCH 141, 618, 626; PEAC 194; department specific 618, 626; other individual studies as needed.

Petition to take Graduate courses for Graduate credit as an Undergraduate student
Undergraduate students, in accordance with the stated guidelines and approvals, may petition to take graduate courses for graduate credit. Note that this policy does not pertain to academic programs whose curriculum design combines undergraduate and graduate requirements such as Health Science/Pre-Physical Therapy.

Graduate Student Petition to register for over 18 credits in a semester
Graduate full-time enrollment is defined as nine credit hours. Students wishing to take an overload must have completed one full term, be in good academic standing with a B average, and have the permission of their academic advisor. More than 18 credits is considered an overload.

Undergraduate Student Petition to register for 21 or more credits in a semester
Students who wish to register for twenty-one semester hours or more must have the written approval of their advisor and the dean of the school in which their major resides.

Transfer Credit Pre-approval form (Undergraduate)
This form should be used by all undergraduate students wishing to complete college credits at another institution to apply toward their degree program at Springfield College. The form must be completed and approved by both the Academic Department and the Academic Advising Office prior to completing the course. It is the responsibility of the student to ensure that all criteria are met prior to registering for the course.

Leave of Absence Request (Graduate)
This form is to be used as an official request to temporarily leave or exit the College with the intent of returning at a later date.

This form is to be used as an official request to withdraw from or leave the College without the intent of returning at a later date.
